If I pay to remove the back link I see it is for only 1 server but does this mean it must stay on 1 domain for the lifetime. For example can I use it for 1 website and then change it to another down the road to another domain and hosting account or would I have to pay again to have the back link removed.

No, you dont have to pay again, you can change domain and host as often as you want. The license is not bound to a domain or machine. The only thing which is not allowed is to use the license on two different servers at the same time.
